People Machine Interface - Unloading Tin

Incident Date: 4/6/19

Incident Description: While unloading tin at the peddlers area,  two customers were observed, #1 on the bed of a truck and #2 on the ground. Customer #1 on the bed of the truck handed a bike from the rear of the truck to customer #2 who then proceeded to mount the bike and ride around the yard near the unloading area. During this period a loader operator was turning to go to the south area of the yard and had to immediately stop as bike-riding customer #2 turned in front of the Loader. No Injuries reported.

Root Cause: Inadequate control of the visitors to the yard

Corrective Action: Additional training to inspectors to make them aware of this unusual possibility with customers.
