Incident title: Flatbed Incident
Incident Type: Transportation Incident
Incident Description: Flatbed truck was getting offloaded in the sort area and when the loader operator attempted to lift the piece it caused the loader to come off its back tires. At this time the flatbed driver pulled away causing the weight of the piece to tip the loader all the way forward and slamming it back down on the rear tires. The operator was uninjured but the potential of a serious musculoskeletal injury could have occurred.
Recommended Action based on potential: The company has reiterated that the failure to follow the clearly established procedures during heavy lifts has the potential for serious incidents to occur.
