Transportation Incident - Fork Truck Storage Rack

Incident Date: Unknown

Incident Description: Fork truck operated attempted to remove a box of cans from a storage rack.  Did not get the forks far enough into the pallet to pick up the load securely. The load caught on the storage rack narrowly missing from upseting the rack.

Corrective Action: Secure the rack to avoid upstting the entire rack. Instruct all to stop and contact supervisor when load is unstable.  Lack of PPE could have resulted in injury.
