People Machine Interface - Forklift Operator

Incident Date: 11/03/17

Incident Description: Fork lift operator started machine did not perform inspection. Parked machine on loading dock plate that was not returned to it’s cradle. Dock plate was sloped. The E Brake was faulty, the fork lift rolled off of the loading dock flipped striking the ground.

Corrective Action: Pie trip and post trip inspections are mandatory. Insure compliance. Create a method of checks and balances to ensure compliance. Review inspection policies with employees. Review equipment PM maintenance with staff. Be certain all safety equipment is fully functional.
